I think this is the way many companies can go in the future. Obamacare is not specific on WHERE companies need to provide heath insurance. As far as I know, there is nothing restricting a large company from opening their own health care clinic or from buying a large part of their healthcare plan upfront from a surgical company you profiled.
For example, I work for a large hospital. Starting this year, we get 100% coverage of nearly all health care services if we do them in-house, and 80% coverage with a 3.5K deductible per person / 7K deductible per family if we choose to do them at an outside hospital. If we are admitted for an emergency situation at an outside hospital, my hospital will pay for a HAL transfer back to our facility. It's not illegal under ObamaCare to do that!
